On July 11th, I called in a survey for Dr. Reid # 321-l04-330. I punched the wrong button for question #20. I most definitely would recommend Dr. Reid to family and friends!!! He is very
caring and thorough. I pushed "no" and I meant to push "yes". I didn't know how to correct

Very good survey. I already sent in the paper survey a short time ago, but in the meantime I got a follow up letter from my doctor reminding me to proceed and so I went ahead to submit my answers online.
